Essay

The work is devoted to the problem of search new effective methods of treatment of trophic ulcers of venous origin. The clinical part of the work is based on a comprehensive examination and treatment of 124 patients with trophic ulcers of venous origin. Patients were divided into two groups: primary and a comparison group. Group comparison of 61 patients whose treatment was carried out by traditional methods used in clinics, without surgery skin plastic. In the main group included 63 patients in whom conventional therapy was supplemented by transplantation of ulcerative defect of skin grafts using local microwave irradiation using our method. An efficiency of incorporation into the complex treatment of local microwave radiation with frequencies of 0,915 and 2,45 GHz patients with trophic ulcers of venous origin in the preparatory phase of the ulcer to the skin plastic, and after autodermoplastic. The clinical effectiveness of the work presented technique of a local microwave irradiation ulcers by local improvement of microcirculation and lymph flow of stabilization, improved tissue oxygen consumption, improved reparative processes in tissues, edema of the inflammatory reaction, reduction of pathogenicity and resistance in the microflora, as well as increasing its sensitivity to antibiotics that allows the shortest possible time to prepare patients for skin or plastic operations on the veins of the lower extremities. Inclusion in the complex treatment techniques of local microwave irradiation of trophic ulcers in the first days after autodermoplastic achieves 80-100% engraftment graft by improving microcirculation and increasing oxygen tension in the tissues in the radiation zone. Developed and tested the simultaneous execution of surgical interventions with the aim of correcting disorders of venous hemodynamic and the implementation of landmark skin plastic closure of trophic ulcers. Conducted training indicate that the use of intravenous injection of ozonized physiological solution with the local use of Curiosin gel; use of the electromagnetic microwave radiation, excision of trophic ulcers and necrectomy in combination with freeze-dried ksenokozhey have advantages over traditional methods of preparation of trophic venous ulcers for surgical intervention, so how they contribute to the shortening of the preoperative preparation of various size and degree of changes of the surrounding tissues of the ulcer
