Dissertation work is dedicated to define the influence of remodeling of heart, ischemic, pressor, vegetative factors, changes of cytokine system on the electric myocardial instability in patients with chronic ischemic heart disease as a precondition for treatment correction. Defined were the criteria of electric instability diagnostics, myocardial arrhythmogenity in patients with ischemic heart disease as a sequel of functional, morphologic, electrical remodeling of atria, ventricle, geometry of depolarisation, repolarisation processes, cytokine exchange, ischemic, pressor, vegetative, age factor, sexual activity influence as a precondition for treatment correction. The criteria of diagnostics of chonic coronary insufficiency intensity were elaborated, electric myocardial instability in patient's chronic ischemic heart disease, depending on type of heart remodeling, predominantly left ventricular, biventricular including cavity dilatation. Specified were the significance of decrease of the level of low-frequency range slow waves of the first order as well as high frequency breathing waves, increase of maximum and mean day diastolic blood pressure in the decrease of adaptation capabilities of the parasympathetic and sympathetic neural system, their "depletion", disadaptation between the contraction ability of the heart and peripheral vascular resistance in patients with IHD and stage 3AH and arrhythmias, focal sclerotic changes in the left myocardium. The algorythms for defining myocardial arrhythmogenity in dependence from neuro-vegetative influences on coronary-myocardial activity basing on vertebral stress-test, stellar nodes in patients with ischemic heart disease combined with cervical-thoracic ostheochondrosis were specified. The combination of multifactor influence of excessive activity of anti-inflammatory - interleukin -1? and/or interleukin -6 and/or interleukin -8 and/or C-reactive protein and/or factor of tumour necrosis ? and decrease of anti-inflammatory cytokines activity interleukin -10, ischemic, pressor, vegetative rhythms on the process of remodeling, electric instability of the heart in IHD patients were defined. The criteria for diagnostics of Moebius-like space orientation of depolarization, repolarization processes by the change of supraventricular pacemaker on ventricular were defined. Specified were the fractal mechanisms of depolarization processes repolarisation of atria and ventricles in patients with IHD combined with sinal node dysfunction in particular by such major types of iteration as substitutional, functional systems, formular.
