Essay

The aim of the dissertation is to improve the efficacy of surgical treatment of BPH by means of working out of the complex of prophylaxis and elimination of the irritative symptoms in patients who have been treated with ?1 - adrenoblocators during a long time before the operation. The presents of the LUTS in patients after the radical operation is proved to be depended on prolonged administration of the ?1 - adrenoblocators before operation. The imperative symptoms are more often presented in patients who were treated with ?1 - adrenoblocators before operation then in patients who have not been treated with these drugs or continued the administration of ?1 - adrenoblocators after the operation. On the basis of this dependence, peculiarities of the dynamics of the restoration of the blood flow in the cystourethral segment after the operation, intensity and duration of hypercatecholemia, the existance of the "discontinuation syndrome" after stopping of administration of ?1 - adrenoblocators is proved in the dissertation. The analogical dependence between LUTS development and administration of blocators of 5-?-reductase was not found. The algorithm of treatment and prophylaxis of the lower urinary tract symptoms in postoperative period was worked out and inculcated. This algorithm decreases the probability of micturition disorders in postoperative period in 1,43±0,11 times, improves the quality of life and approximates the functional parameters of micturition to the physiological norm in 92,57±3,4'% cases.
