The thesis is dedicated to optimization of diagnostic and treatment approaches in patients with chronic brain ischemia on the ground of complex clinico-instrumental research, study of hormone-metabolic and hemodynamic parameters, pathogenetic validation of drug tiocetam. Clinical course of dyscirculatory encephalopathy was studied, 5 syndromes were revealed: cerebrastenic, vestibule-cerebello-ataxic, pyramidal insufficiency, cephalgic, mild cognitive impairment with predominance of dysmnestic component. Increasing of resistance and pulsation index revealed in the pool of anterior and posterior cerebral arteries, which is the evidence of angiospasm in the zone of co-operating circulation, and correlates with the intensity of cognitive impairment in patients with chronic brain ischemia. The role of nitric oxide, carbonil proteins, activation of protein peroxydation in pathogenesis of chronic brain ischemia was proven. Importance of increasing of melatonin's biosynthesis in realization of antioxidant action was determined. The effectiveness of tiocetami in complex treatment of dyscirculatory encephalopathy was demonstrated. It improves clinical symptoms, cerebral hemodynamic parameters and decreases activity of protein peroxydation.
