Essay

This work was dedicated to the determination of role of systematic and local (in decidual tissue) matrix metabolism of connective tissue during preeclampsia pathogenesis. Analyse of such phenotypes as familiar, somatic and reproductive anamnesis allows to correlate appearance of preeclampsia with insufficient of connective vessel matrix and decidual tissue. For preeclampsia disorder as exchange in main components of connective matrix is common. Processes of collagen, proteoglycan, glycosaminoglycan destruction with further its excretion with urine are prevailed. It is already stated that pathogenesis of endothelial dysfunction of typical disorders in vessel-thrombocyte gain of homeostasis and as follows the hemodynamic disorders in fetoplacental complex are closely connected with insufficient of chondroitin-4-sulfate and heparan sulphate in endothelial glycocalix. Morphological structure of decidual membrane during hestosis is characterised in pronounced dystrophic and necrotic processes, and also with massive cells apoptosis, insufficient of CD8 lymphocytes on the background of relative increase population of CD4 reflected in NK-cells deficit (CD-56) and in increased amount of ИЛ-1?-procedures. Deficit of vessel component of decidual membrane during gestosis accompanied by the raised level of receptors expression to the endothelium-1, that is could be interpreted as local dystrophic and necrobiotic processes and also as systematic disorders in hemodynamics. Connective matrix of decidual tissue is dominated with destructive processes and misbalance in fractional composite of glycosaminoglycans. Didrogesteron allows with the decidual endometria to generate specific conditions for synthesis of decidual cells components of connective matrix, provide immunosuppression in decidual tissue and reduce production of proinflammatory cytokines that further will lead to gravidoprotection.
