Subject of inquiry: - wound processes, flowing in a chronic posttraumatic ulcer; The aim of the research: improvement the results of treatment of patients with the posttraumatic trophic ulcers of lower extremities due to development and introduction of new methods of diagnostics and surgical tactic; Methods - clinical, morphological, microbiological, instrumental, statistical; Scientific novelty: it is showed that morpho-functional changes developing in fabrics of the posttraumatic ulcers of lower extremities are universal and does not depend on etiologic reasons, localization, area and depth of defect of fabrics; it is well-proven that microbiological picture, citomorphological and microcirculation changes, developing in fabrics of posttraumatic trophic ulcers in the stage of chronic inflammation, carry universal character and does not depend on etiology of exogenous damaging agent; distinction of superficial and deep layers of posttraumatic trophic ulcer is scientifically grounded what principles and algorithm of surgical treatment of chronic posttraumatic trophic ulcers of lower extremities are developed on the basis of, which allows expressly to define tactic of surgical treatment of posttraumatic trophic ulcers depending on the depth of ulcerous process, his localization and area of defeat of fabrics; classification is developed chronic trophic ulcers of lower extremities as it applies to posttraumatic ulcers, which along with the depth of damage of fabrics takes into account localization and area of ulcer and allows to ground tactic of surgical interference; Results: the use in treatment of patients with the chronic posttraumatic ulcers of lower limbs of the worked out algorithm of surgical treatment with early surgical delete of ulcerous defect and early plastic closing of wound defect allowed to reduce the terms of stationary treatment in the specialized separation on 4,9 ± 1,7 days (р < 0,05) at the ulcers of II degree, on 7,3 ± 2,1 days at the ulcers of III degree (р < 0,05) and on 9,8 ± 1,9 days at the ulcers of IY degree (р < 0,05), to decrease general duration of stationary treatment on 25,2 ± 4,5 days (р < 0,001), in 2 times to increase frequency of complete to get accustomed of autodermotransplantats, in 3,4 time to shorten the amount of the repeated operative interventions and in 2 times to decrease frequency of relapses of ulcerous process; It is inculcated - in medical practice, in an educational process; Sphere - medical practice.
