Essay

The work deals with the problem of decrease of recurrent hyperproliferative processes of endometrium by means of improving methods of its treatment on the basis researching associations between endometrial morphology, presence of genitalia inflammation, vaginal microbiocenosis type, hormonal homeostasis and adipose tissue percent. It was found that in 57 % of women of late reproductive age with chronic salpingo-oophoritis endometrial hyperplasia originates from chronic endometritis. Research of CD 95 Fas receptors' expression showed that severity of endometrial hyperplasia depends mainly on the activity of general and local inflammatory factors which define the balance between proliferation and differentiation of endometrial tissue. The results of microbiologic investigations demonstrated important role of viral and bacterial flora in the development of endometrial polyposis and atypical forms of endometrial hyperplasia, while their combination with the decrease of progesterone receptors' expression in epithelium and stroma place inflammation into the first line of endometrial hormone resistance risk factors. Suggested therapeutic complex for patients of late reproductive age with endometrial hyperplasia based on chronic salpingo-oophoritis which includes progestational agents in combination with etiotropic antibacterial and nonspecific anti-inflammatory and antiviral therapy, аs well as recommendations on body weight normalization showed significant clinical effect in the aspect of recurrence prevention and endometrial structure normalization.
