Essay

The object of the research is ischemic heart disease (IHD) in patients who undergo coronary stenting (CS). The purpose is to improve treatment efficacy and control of the IHD course on the basis of clinical and functional indices analysis and quality of life assessment before and after CS. The methods are clinical, laboratory, instrumental, sociological and statistical. The novelty of the research lies in the following: it has shown that CS in patients with IHD allows to improve their response to treatment for a long time; has proved that complex therapy of IHD involving medical and interventional treatment combined with compulsory training of patients and their family members and constant control of the disease course is the most effective; has established that CS helps to improve tolerance to physical exertion as well as quality of life and ejection fraction of the left ventricle while preserving its dilatation without any significant deterioration of functional status of kidney; treatment of IHD with CS involvement in patients with dyslipidemia, arterial hypertension and postinfarction cardiosclerosis promotes significant improvement of the disease course; on the basis of ultrastructure study of biopsy and necropsy material unchanged, hibernate and stunned cardiomyocytes and areas with their necrosis and apoptosis have been found in myocardium under ischemia. The research has achieved the results: 1) arterial hypertension, family history, and contacts with xenobiotics are the most popular risk factors in patients with IHD who undergo CS; 2) CS allows to improve tolerance to physical exertion, ejection fraction of the left ventricle, quality of life, and response to treatment; 3) CS doesn't affect functional status of kidney; 4) CS helps to improve IHD course in patients with dyslipidemia, arterial hypertension and postinfarction cardiosclerosis which has beed proved by dynamics of the indices to the tolerance to physical exertion, ejection fraction of the left ventricle and quality of life; 5) CS has been the most effective in patients with postinfarction cardiosclerosis; 6) there are unchanged, hibernate and stunned cardiomyocytes as well as areas with their necrosis and apoptosis in myocardium under ischemia. The results of the research have been introduced into medical institutions, scientific and educational process. The field is Medicine.
