Essay

The results of retro- and prospective analysis of surgical treatment of 148 patients with mechanical jaundice of non-tumor and tumor genesis are presented. The analysis of techniques for decompression of biliary tract, identified the priority, the indications and contraindications to their implementation, depending on the nature of pathology and level of biliary tract obstruction was made. The complications after antegrade endobiliary interventions were standardized and divided into groups - a group of severe and light complications. The causative factors of complications associated with deficiencies of biliary tract decompression methods, primarily with the Seldinger technique, were identified. The methods of antegrade endobiliary interventions were developed and improved, thus reducing of complications frequency of draining operations from 29,4 to 14,4%. Stenting of biliary tract can be finale manipulation as alternative reconstructive operations. Processed and implemented in clinical practice antegrade endobiliary interventions algorithm in patients with mechanical jaundice of non-tumor and tumor genesis, considering the causes of jaundice, biliary block level, the general condition of the patient and assessment of hepatic functional reserve allowed to significantly improve treatment results, reduce postoperative complications frequenct and mortality.
