This dissertation is devoted to the investigation of actual problem of modern cardiology - feasibility study of irbesartan use in patients with paroxysmal and persistent arterial fibrillation, improved clinical - genetic aspects of research in this disease. Proven link between I/D polymorphism of angiogenesis-converting enzyme gene in patients with arterial fibrillation affects not only the clinical course of disease and extent of myocardial expression of structural changes, but also on the level and effectiveness of therapy MNUP blocker of the renin-angiotensin system - irbesartan. Determination of angiotensin-converting enzyme gene polymorphism in patients with arterial fibrillation allows to select high risk group among patients for the appointment of more effective treatments and targeted prevention measures of complications of AF, and differentiated approach to the therapy, preferring in the media I - allele- irbesartan. The positive effect on the reduced level of investigated peptide if the presence of heart failure, hypertension and coronary heart disease and CAD is proved. Lack of significant changes ВNUP patients with AF in the presence of diabetes and obesity is reduced effectiveness of irbesartan treatment.
