Essay

Object: ejaculate, spermatozoids, embryos of the 2nd-6th day of development. Aim: To develop the system of sperm morphofunctional quality assessment in case of human spermatogenesis pathology for further application of the selected cells in extracorporeal fertilization. Methods: cultural, cytological, methods of molecular biology, cytogenetic, statistical. Results: IMSI (intracytoplasmic morphologically selected sperm injection) method was developed to improve effectiveness of male factor infertility treatment. IMSI based on real-time motile sperm examination and selection for in vitro fertilization under magnification up to x 6300. Analysis of the generated data proved effectiveness of morphofunctional sperm selection under x 6300 magnification for patients with teratozoospermia and oligozoospermia. Effectiveness of sperm selection under x 6300 magnification was not proved for males with asthenozoospermia. Morphofunctional sperm selection allows significantly increase blastulation rate. Correlation between sperm morphology assessment under x 6300 magnification and Kruger's strict criteria was determined. Morphofunctional sperm selection could be used to improve efficiency of infertility treatment. Sperm selection under x 6300 magnification could prevent vertical transfer of existent and de novo formed mutations by rigorous selection of the sperm for fertilization in vitro. Novelty: Morphological features of sperm using differential interference contrast microscopy were first characterized in case of human spermatogenesis pathology under x 6300 magnification. Direct association was revealed between the number of normal spermatozoids, results of in vitro fertilization and further development of embryos in vitro. Efficiency of embryo in vitro cultivation using hyaluronic hydrogel and morphofunctional sperm selection was demonstrated. Methodical system of micromanipulation, analysis and sperm selection under x 6300 magnification was elaborated. Morphofunctional sperm selection could be used to improve efficiency of infertility treatment for patients with increased level of aneuploidies in reproductive cells.
