Snisar A. Medical-social assessment of results of treatment of complicated ulcer disease and possibilities of surgical rehabilitation of patients with limitations of vital activity

Dissertation is devoted to the problem of medical-social rehabilitation of patients with limitations of vital activity after surgical treatment of ulcer disease (UD) and its complications. Data of 397 patients with gastric UD and duodenal UD have been analyzed, of them 350 patients underwent surgery on UD and its complications. Leading causes of development of unsatisfactory disabling consequences of surgical treatment of UD and its complications is an increased gastric acidity, contamination with H. pylori, disorder of motor-evacuating gastric and duodenal function, pathologic bile reflex and dumping-syndrome. An improved technology of medical-social rehabilitation of this category of patients and prognosis model were proposed. Due to this model it is possible with a high specificity (95,2%), sensitivity (84,9%) and correctness (86,3%) to prognosticate probability of disability development after UD surgical treatment. Choice of pathogeneticaly justified means of surgical rehabilitation allowed to reduce number of UD relapses from 51% up to 9,1%.
