The dissertation is devoted to the problem of working out and substantiation of differentiated programs of surgical treatment of patients with hand burns, efficiency of application of early surgical interventions (ESI), performance of flap plastic with the axial type of blood supply, island flaps based on perforant arteries. It was found out that application of ESI promotes more effective restoration of active movements in joints in 1. 29-1. 33 times (р<0. 04), prevents development of hand contractures and improves functioning of the organ after the burn, promotes restoration of the basic captures in 1.56-1.70 times (р<0.002). ESI increases subjective index of satisfying of personal life requirements in patients with third- degree burns by 8.89 points (10%, р<0.02), with fourth-degree burns - by 14.28 points ( 11.1%, р=0.01). Life quality in patients with second-degree burns increased - 96.65±3.69; with third-degree - 97.08±1.32; with fourth-degree burns - by 91.56±3.20 (р<0.01). Application of island flap plastic on vascular leg promoted more active restoration of active movements in 1.28 - 2.25 times in comparison with alternative methods of plastics (р <0.01), led to full restoration of basic captures in 6 months after the trauma, normalisation of haemodynamics indices: Vmaxs =32.48±0.76cm/with, RI = 0.86±0.04, PI = 3.94±0.13 (p <0.001). Introduction of the program has reduced the average hospital stay of a patient in 1.2 - 1.8 times (p <0.001), has decreased frequency of cicatrix and contracture development in 2 times: from 26.9 % to 11.6%(р<0.01).
