Baydan V. Videothoracic surgeries in the diagnosis and treatment of complicated traumas of chest

Dissertation work is devoted to the pertinent problem of surgery - increasing effectiveness in the diagnosis and treatment of the patients with complicated traumas of the chest with the use of videothoracoscopic technologies. In the work, differential characteristics of endoscopic operations and traditional methods (pleural effusions, pleural drainage and wide thoracotomy), has been illustrated. The advantages of endoscopic methods have been approved. The step-by-step methods of videothoracoscopy, technical aspects of their use, recommendations in the post operative period and the principles of prophylaxis of intra-operation complications have been developed and optimized. The criteria for the videothoracoscopic and video assisted operations have been defined. Use of treatment and diagnostic tactics gave the possibilities to perform operative manipulations in 99 out of 110 patients endoscopically, decrease in the chances of thoracotomy by 4.2 times, the post operative complications by 4.9 times, additional drainage of pleural cavity and pleural effusions by 5.7 times, decrease the hospitalization period from (11,5 ± 0,7) to (9,6 ± 0,8) days.
