Pashayev V. Diagnosis and surgical treatment tactics of benign obstructive disease of the terminal common bile duct.

The dissertation is devoted to optimize the treatment process for the patients with complicated benign obstructive disease of the terminal common bile duct (TCBD). Based on in-depth study of morphofunctional changes in the wall of TCBD in his obstruction developed surgical and medical-diagnostic algorithm for selection of surgical intervention in patients with obstructive disease TCBD, improved existing, developed new original methods of diagnosis and surgical treatment of patients with complicated obstructive disease TCBD. The group of patients with degenerative changes in the wall of TCBD against severe inflammation, high and long mechanical jaundice, which proved the expediency of the application is transpapillar interventions to prevent early and late postoperative complications and repeat surgeries. Also found was a group of patients with no degenerative changes in the wall of TCBD against absence of inflammatory processes at low and acute jaundice, which use transpapillar intervention was not necessary if recovery bile flow otherwise. Thanks to a differentiated approach to the choice of surgical intervention including morphofunctional disorders in TCBD in its benign obstruction, development of new methods of diagnosis and treatment could improve treatment results, reduce postoperative complications by 8.5%, reducing the number of repeat surgeries to 17.9% , to avoid mortality, reduce the risk of late complications, including postcholecystectomy syndrome, the data of patients by 20.5% due to the active application transpapillar surgery.
