The dissertation is aimed at solving the actual problem - to objectively assess the reserved potentials of kidney in long termed (above 3 months) obstruction in upper urinary tracts and to choose the rational tactics to treat by draining the latter by means of applying the percutaneuos paracentetic nephrostomy and dynamic following the number and specific gravity of urine received through nephrostomy 4-6 weeks later after applying the latter.Based on the assessment of the common methods for diagnosing the renal function in the patients with hydronephrosis and ureterohydronephrosis in terminal (III A - B) stage, there was worked out the own approach for diagnosing the functional reserve of an injured kidney by using percutaneous paracentetic nephrostomy, that allowed to elevate the number of the organ preserving operations in 81.9% of adult patients and in 76% of children.There was substantiated and proposed a new treating and diagnosing algorithm, the key element of which is percutaneuos puncture nephrostomy that provides the accurate determination of the reserved ability of a kidney and, in such, to objectify the rational choice of a method for surgical intervention. There is recommended the simplest and informative method to assess the functional ability of an injured kidney.
