Essay

The study is devoted to improving the long-term results of surgical treatment acute necrotizing pancreatitis with the use of endovideoscopic technologies. Clinical studies on 182 patients proved that the metod developed landmark endovideoscopic necrectomy allow to fully carry out necrosectomy and is characterized by significantly less than open necrosectoy, degrees of pain, severity of stress reaction (according to the level of plasma cortisol - 470.3 ± 7.4 mg/l vs 612.9 ± 9.6 mg/l, respectively) and is not accompanied by the release into the general circulation of microbal metabolites. Minimizing the surgical trauma o the developed technique is assotiated with a lower incidence of MOD (10%), compared with a standart (52%), (р < 0.001). An improved algorithm for the treatment of patients with acute necrotizing pancreatitis with the use of original methods in comparison with the traditional approach provides a significant reduction in mortality postoperative on 19.1% and the frequency of postoperative complications: intestinal fistula on 28.8%, postoperative inflammation of the wounds on 44.8%, hernia anterior abdominal wall on 31.9%.
