Nechay V. Ways of improvement of results of surgical treatment of patients with chronic haemorrhoids III-IV stages

Disertation is sanctified to development and introduction of new diagnostic technologies, to the improvement of methodologies of implementation of surgical interference with treatment of chronic haemorrhoids of ІІІ - ІV stage. The basis of this study are fixed results of inspection, treatments and looking after 468 patients with the chronic haemorrhoids of ІІІ - ІV stages. The prospective analysis of results of surgical treatment of the patients operated under the method of the closed hemorrhoidectomy and standard stepled haemorrhoidopexy by Longo is conducted. The features of angioarchitectonic of distal branches of overhead rectal artery are certain the method of intraoperative sonografy. On the basis of the conducted analysis of the obtained data the method of modifficative stepled haemorrhoidopexy is worked out, that consists in sonografy determination of level of forming of internal circular guy-sutures, additional ligating of guy-sutures of vascular legs of haemorrhoidal knots in area of of circular stepled guy-sutures and realization of additional haemorrhoidopplication at the insufficient replicon of prolapce of anal mucose for patients with the ІV stage of chronic haemorrhoids. Offered method modified stepled haemorrhoidopexy allowed statistically for certain to improve both direct and remote results and functional results of surgical treatment, improve quality life and reduce the terms of rehabilitation of the operated patients, obtain absence of relapse after surgical treatment.
