Babych V. Optimization of surgical treatment of acute perirectitiss.

Work is devoted to the improvement of direct and remote results of holiatry of patients with a acute perirectitis (AP) by the improvement of methods of diagnostics and individualized tactics of surgical treatment and conduct of patients in a postoperative period. It is set that the results of surgical treatment of AP depend on timely, exact topical diagnostics of abscesses of perirectal cellulose, adequate to the executed operative intervention and rational conduct of wounds of crotch in a postoperative period. Complications in diagnostics of deep forms of AP meet in 29% cases. A computer tomography allows in difficult and doubtful cases to bring down the number of unreliable results of research in 3 times (from 12,7% tо 4%). Differentiated going near the choice of optimal method of operative intervention depending on complication of AP in combination with application at treatment of wounds of crotch after radical dissection of AP of methodologies local SHF-irradiation with frequencies 2,45 and 0,915 GHz allowed to attain more early normalization of the state of cellular and humoral immunity, promote efficiency of antibacterial therapy and shorten duration I and ІІ of phases of wound process. The worked out curatively-tactical approach at a sharp perirectitis allowed to shorten terms of treatment of patients from 33,06±3,15 to 15,8±2,06 days, substantially to bring down frequency of early postoperative complications from 13,6% to 3% development relapses of disease and transition of process in chronic motion from 11,8% to 4% on comparison with the traditional methods of treatment.
