Dissertation is devoted to the problem of determining the state of cerebral blood flow among patients with uncontrolled arterial hypertension based on the study of clinical and instrumental parameters for the prevention of hypertensive crises and treatment elaboration. The leading neurological syndromes: cephalgic, vestibular-cerebellar, pyramidal where established. Was founded that the trigger factors of hypertensive crisis among UAH patients are psycho-emotional overstrain, taking only the one kind of antihypertensive drug, antihypertensive drugs omitting. As a results of ambulatory blood pressure monitoring on the researching ground was found load indices increase on the cardiovascular system by reducing the variability and blood pressure morning surge performance in the hypertensive crisis progress, non-dipper and night-peaker circadian dysrhythmia. Marked reduction of the cerebral blood flow and the cross-cerebellar diaschisis presence among chronic cerebral ischemia with uncontrolled hypertension patients was founded. Reduce of cerebral blood flow due to increasing of the pulse pressure and morning blood pressure indicators intercommunication has been elicited. New approaches of antihypertensive therapy combination among patients with UAH, depending on the ratio of cerebral hemodynamics against the background of the chronic ischemia, which included the achievement of target blood pressure and improve of the brain perfusion, where developed.
