The purpose of the research was to decrease the level of obstetric and perinatal complications in women with diseases of the thyroid gland by improvement of efficiency of the pathogenetically grounded criteria of diagnosis, prophylaxis depending on the features of the congenital predisposition of a woman. The thyroid gland pathology rate for pregnant women in the Odessa region during 2001-2010 was 22.5%o in 1000 births. Early toxicosis, preeclampsia, placental dysfunction, syndrome of intrauterine growth retardation of fetus, chronic distress of fetus, gestational anaemia, obstetric bleeding were main gestational complications. Iodine level in urine was lower than in the control group 3.4; 3.0; 3.1 times on the gestation trimesters (p<0.05), frequency of mutant alleles of endothelial NO-synthase 4a/a was 70.0%, concentration of thyroid-stimulating hormone and antibodies to thyroidperoxydase exceeded 1.5; 1.8 times as much with decrease of total triiodthyronine and free thyroxin - 2.2; 1.9 times (p<0,001), progesterone level - 1.2 times. Application of the offered measures assisted decrease of clinical displays of abortion threat 8,1 times, preeclampsia - 3.0 times, gestational anemia - 2.1 times, placental dysfunction - 3.7 times, syndrome of intrauterine growth retardation of fetus - 3.7 times, chronic dystress of fetus - 5.7 times, pathological blood loss - 3.5 times as compared to the generally accepted treatment.
