The scientific work is devoted to actual problem of diagnosis, clinical flow and treatment of surgical gramnegative sepsis in patients with suppurative-inflammatory processes of soft tissues. The results of examining and treatment of 130 patients with surgical sepsis were summarized. First group formed of 78 patients with suppurative-inflammatory processes of soft tissues, which had gramnegative sepsis. Second group formed of 52 patients with suppurative-inflammatory processes of soft tissues, which had grampositive sepsis. Principal etiological factors to development of surgical gramnegative sepsis were established. It were Ps. Aeruginosa and E. coli. The number of cases of gram-negative sepsis during the study period decreased 1.2 times.Clarified the clinical picture and symptoms in patients with gram-negative sepsis, permitting effective differential diagnosis of gram-negative sepsis with gram-positive form of the disease.Study the dynamics of the sensitivity of gram-negative flora to antibiotics indicates the growth of antibiotic resistance to most of the the means used, and there is no alternative to the use of carbapenems. In patients with severe Gram-negative sepsis and its complicated forms, there are significant changes in lipid metabolism, and initially elevated triglyceride levels may be to some extent be considered as a marker of poor outcome.
