The thesis is devoted to the solution of a scientific problem - reducing the incidence of gestational and perinatal complications in pregnant women with preeclampsia with pathology of the hepatobiliary system through the development and implementation of science-based complex treatment and preventive measures. It is proved that in the pathogenesis of preeclampsia the significant role is played by the endothelial dysfunction. The presence of the endothelial dysfunction in women with preeclampsia together with the hepatobiliary pathology is confirmed by the increase of vasoconstrictor ET-1 in 1,2 times (p<0,05), decrease of vasodilator NO in 3 times (p<0,05) in the serum. The imbalance between vasoconstrictors and vasodilators confirms their pathogenic role in the development of preeclampsia in pregnant women with the hepatobiliary pathology. Introduction of the therapeutic and preventive measures led to a decreased risk of: preterm labor - 3 times; placental insufficiency - 1,8 times; fetal growth retardation - 2,2 times; preterm birth - 5 times; cesarean - 4 times; adult respiratory distress syndrome - 3 times; intrauterine growth retardation - 4 times (p<0,05).
