Thesis is devoted to the problem of improving the diagnosis and treatment of patients with bleeding pyloroduodenal ulcers. Refined risk factors of recurrence of ulcer bleeding: increased tumor necrosis factor> 22.4 pg/ml indicates a risk of adverse outcomes flow ulcer bleeding and reduction of 2 or more times during the treatment of tranexamic acid - an indicator of the effectiveness of hemostatic treatment. In the early posthemorrhagic period predictors of recurrent bleeding is increasing TNF > 22.4 pg / ml, pepsinogen -1 > 165 mg/ml, pepsinogen -2 > 35 mg / ml, gastrin > 100 mg / ml. Proven benefits of treatment with high doses of pantoprazole 80 mg/bolus followed by an infusion of 8 mg/hour and tranexamic acid 10 mg/kg/day, by reducing the amount of recurrent bleeding, operations performed at relapse, and overall postoperative mortality.
