Essay

The thesis studied the dependence between daily profile blood pressure, variant of polymorphism gene angiotensin-converting enzyme, activity of angiotensin-converting enzyme and severity of patients with different pathogenic subtypes of ischemic stroke. It is show that the most severe stroke is observed in patients with genotype DD, accompanied by the most high level systolic blood pressure and diastolic blood pressure during the day and much higher level of activity of angiotensin-converting enzyme activity compared with other genotypes. Patients with II genotype have the best functional outcome and better performing daily monitoring of blood pressure. Revealed that the level of activity of angiotensin-converting enzyme correlate with the level of blood pressure. Applying of angiotensin receptor blockers olmesartan significantly reduced the mean blood pressure and reduce morning rise and degree of night blood pressure reduction. Olmesartan has a more pronounced effect on night hypertension and decreased activity of angiotensin-converting enzyme than other classes of antihypertensive drugs.
