Maloshtan A. Surgical tactics and treatment of patients with single liver abscesses.

On the base of analysis of treatment of 140 patients with the single pyogenic liver abscesses in dissertation theoretical generalization and practical decision of scientific task are executed. The results of surgical treatment of patients with the single pyogenic liver abscesses, due to the differentiated use of miniinvasive and resection methodics of treatment and the morphological state of abscess, are improved. In the experiment on 30 pigs the local inflammation reaction is studied, due to using of different technologies of liver parenchyma dissection (clamp crushing, classic and ultrasonic scalpels, water- and pneumojet dissectors). It's proven that jet technologies cause less local inflammation reaction. That's why they are most suitable for application at resection methodics of treatment in the conditions of the potential infecting. In the research large attention is spared to the study of single pyogenic liver abscess wall transformation during its forming (from 10 days to 4 month). The division of acute abscess period, which is based on the morphological changes of abscess wall during its forming, is offered: the stage of cavity forming (up to 10 days), stage of capsule forming (10-30 days) and stage of perisclerosis (1-3 months). Expediency of the differentiated use of punction-aspiration-drainage and resection methodics of treatment is substantiated, depending on the stage of abscess forming. It's offered the original method of economy resection - "Fenestration".
