Dissertation is devoted to the substantiation of surgical treatment in patients with acute pancreatitis using laparoscopic techniques with enteral insufficiency correction. The results are based on experimental and clinical data. They form a new treatment policy in patients with severe destructive acute pancreatitis which includes laparoscopic mіcroeyunostomy and early enteral feeding. Con-ducted experiment confirmed staging of pathological morphological processes into pancreas during experimental acute pancreatitis progress: edema - till 6 hours, hemorrhage - from 6 till 12 hours, necrosis - after 12 hours. Enteral feeding of ex-periment animals using mіcroeyunostomy reduced pathological morphological processes into pancreas, liver, small bowel. Clinical part included retrospective and prospective study of 513 patients with acute pancreatitis. New technique of laparo-scopic mіcroeyunostomy was proposed and adopted. The employment of new treatment policy with laparoscopic mіcroeyunostomy and early enteral feeding re-duced mortality till 11,1 % for destructive - necrotic forms of acute pancreatitis.
