Dissertation is devoted to the topical question of modern obstetrics - decreasing frequency of lactation disturbances in women with leiomyoma of uterus on the ground of detection of characteristics of endocrine regulation of lactation and determination of pathogenetic factors of lactation disturbances and working out the complex of therapeutic preventive measures. As a result of complex researches in puerperants with leiomyoma of uterus who had spontaneous labor and who had been cesarean operated the disturbances of lactation in the period of its formation are detected. More considerable lactation disturbances are determined in puerperants who delivered by cesarean section. Main pathogenetic factor of lactation formation disturbance in puerperants with leiomyoma of uterus is determined: imbalance of steroid hormones (estradiol, progesterone) and prolactin. Pathogenetic component of lactation disturbance in puerperants with leiomyoma of uterus who have been cesarean operated is temporary impossibility to contact newborns with breast. This delays "starting" the neuroendocrinal reflexes of lactation formation. Increased activity of pituitary adrenal system (rise level of stress-realizing hormones) and insufficient thyroid state are determined as pathogenetic factors of formation of lactation disturbances. Criteria of early diagnostics of lactation disturbances and criteria of endocrine regulation disturbances of formation lactation are established in puerperants with leiomyoma of uterus. Complex of therapeutic preventive measures in puerperants with leiomyoma of uterus is worked out. The complex promotes improvement of endocrine regulation of lactation and decreases frequence of lactation disturbances and enlargement of its duration.
