In this dissertation thesis the solution of actual scientific task such as characterization of acute heart failure (AHF) syndrome patients with ST-elevation myocardial infarction (STEMI) with preserved left ventricular (LV) function using clinical, instrumental and laboratory assessment was elaborated. It was determined that patients with AHF are older, they have predominantly anterior-lateral MI, more structural and functional MI complications, steady restrictive pattern of heart haemodynamics and more frequent development of new MI of cardiovascular death (CVD). Patients with persistent or late AHF have increased electrical heterogeneity of depolarization, disorders of autonomic regulation of heart rhythm and decreased heart rate variability (HRV). Main risk factors (RF) of disease complications were established. In AHF patients main independent RF of three-years CVD were heart remodeling and HF progression underlying factors. In AHF-free on admission day patients main CVD RF were mainly ischemic. Late and ineffective reperfusion, decreased HRV and renal dysfunction.
