The aim was improve early and long-term results and psychosocial adaptation in F-M transsexual patients after sex reassignment surgery (SRS) by developing and introduction of new surgery methods and improvement of their stages. A new solution of important scientific problem for surgical treatment of F-M transsexuals was observed in this article. F-M transsexualism is associated with a significantly greater frustration and low quality of life level compared to men and women of general population. The possibility of positive impact on quality of life and frustration level by sex reassignment surgery in F-M transsexuals was demonstrated. Two-staged sex reassignment surgery was improved and helps to limit the number of surgical intrusions on the external genitals. First stage included masculinization mammoplasty, hysterectomy and appendectomy for all patients, the second stage - neophalloplasty - was performed only in 11 patients. The choice of masculinization mammoplasty method using developed algorithm provides the differentiated approach to treatment and good results in 97,1 % patients. Frustration level analysis allowed developing selection criteria for second stage of SRS in F-M transsexuals. Neophalloplasty technique using groin flap devoids cosmetic defects and provides a reliable positive change in frustration level.
