Essay

Object of study - psoriasis. The aim of study - to improve the efficiency of the treatment of psoriasis with vitamin D deficiency by developing a method of treatment based on the study of vitamin D-dependent mechanisms of disease progression. Methods: clinical, biochemical, immunological, statistics. The thesis is devoted to studying the peculiarities of psoriasis pathogenesis in patients with vitamin D insufficiency/deficiency. Among the Crimean region habitants diagnosed with psoriasis during fall-winter period in 62 % of cases vitamin D-insufficiency/deficiency of two types has been revealed: decrease of the level of vitamin D pre-hormone form (25(ОН)D3) in blood serum and decrease of the level of vitamin D3 active metabolite - D-hormone (calcitriol) 1.25(OH)2D3. It has been revealed that in psoriasis patients during fall-winter period D-deficit is the risk factor of increase of imbalance of local cytokine potential, characterized by increase of synthesis of IL-1beta, TNF-alpha и TGF-beta1 cytokines, induced by keratinocytes and mononuclear leucocytes. Clinical efficiency of combined usage of vitamin D substitute therapy and ultraviolet irradiation in fall-winter period in psoriasis patients with D-insufficiency/deficiency.
