The thesis is devoted to optimizing the management of pregnancy and childbirth in women with operated cervix based on the study of risk factors for complications of pregnancy In this group of patients and establishing relationships with the method of surgery on the cervix a history of vaginal microbiocenosis and bacteriological contamination of amniotic fluid that morpho-functional condition of genitals. Based on a retrospective analysis found that the number of births to women with cervix operated in the last 5 years was increased more than 2.5 times (from 4.8% to 10.4%). The use of modern methods of minimally invasive treatment of pathological conditions of the cervix can afford to reduce the number of complications during pregnancy by 1.7 times. Study of peculiarities of pregnancy and childbirth in women with operated cervix compared with healthy women allowed to identify the critical periods of a threat of termination of pregnancy and cervical incompetence, which made it possible to conduct the treatments and preventive measures for the prevention of miscarriage, premature rupture of membranes and premature birth, and concluded that laser and radiowave surgery vapоrizatsiya more sparing methods, which almost doesn't negatively affect current pregnancy and childbirth. This enables them to recommend treatment of cervical pathology before pregnancy, especially the first. Based on data from a comprehensive study of morpho-functional state of operated cervical, vaginal microbiota, characteristics of pregnancy in these women, the algorithm of pregnancy and childbirth and the data offered by the high efficiency of preventive and therapeutic management of women with cervical operated in anamnesis.
