Essay

Prognostic significance of QTc interval duration in the control, programming, and medical management of patients with pacemakers and cardiac resynchronisation devices is studied. It was found, that stimulation can both lengthen, mainly for single and dual-chamber pacemakers, and shortened, mainly for cardiac resynchronisation devices, the QTc interval duration. It was established, that prolonged QTc interval duration after pacemaker and cardiac resynchronisation implantation at various stages associated with greater frequency and more severe cardiovascular disease and syndromes, worse dynamics of functional parameters of blood circulation and stimulation parameters. It has been shown that cardiac pacing and cardiac resynchronization therapy has not been canceled, but modifying drug therapy, taking into account, among other factors, QTc interval duration. It has been proved that prognostically significant criteria for effective control and drug management of patients after pacemaker implantation with physiological QTc interval duration is chronic heart failure functional classes, doses of anticoagulants and antiplatelet agents, beta-blockers, with a long QTc interval duration - degree of hypertension, the proportion of atrial flutter, atrial fibrillation and ventricle tachycardia, chronic heart failure functional classes, QTc interval duration, end-diastolic volume, doses of anticoagulants and antiplatelet agents, amiodarone, beta-blockers, statins.
