Essay

Dissertation is devoted to the development of criteria for clinical, morphological, immunogistochemistry (IGC) and genetics diagnosis of proliferative benign breast dysplasia (BBD) to justify the surgical method of treatment. For the first time we studied the distribution of allelic variants of the gene polymorphism EsRalfa PvuII in patients with proliferative form BBD. Found association clinical, IGC predictors of proliferative BBD with molecular genetics features of PvuII gene polymorphism EsRalfa. It is proved that the carriers of the minor allele of C/C polymorphism of the gene PvuII EsRalfa tend to severe proliferative and atypical changes in breast tissue. It is shown that in the development of proliferation important role is EsRalfa expression level, due to the presence of the minor allele of a C/C gene polymorphism PvuII EsRalfa. It is proved that the genotype C/C at EsRalfa gene PvuII polymorphism associated with increased expression EsRalfa and the degree of proliferation in breast tissue. It is proved that the combination of clinical predictors of proliferative breast dysplasia (mastalgia, extended period of menstruation), reducing BMI with homozygous C/C polymorphism of the PvuII gene EsRalfa surgical treatment followed by test of EsRalfa expression. On the basis of the criteria developed by the updated and improved algorithm for management of patients with BBD.
