The thesis is devoted to actual problem of modern obstetrics - reducing the frequency of late gestosis complex by improving preventive measures based on the study features state of vascular endothelial homeostasis in pregnant women at high risk of occurrence preeclampsia and pathogenic effects on violations. Complex studies showed that at high risk of preeclampsia in those women who later realized preeclampsia, already accepted for the second half of pregnancy marked by violations of hemostasis (thrombocytopenia with anisocytosis and increasing platelet aggregation, increasing the number of fibrinogen, decreased APTT, rising D-dimer), the presence of endothelial dysfunction (reduced levels of L-arginine at high levels of homocysteine), carriers of a mutant variant polymorphism 4b/4a gene eNOS, imbalance processes of angiogenesis and apoptosis (decrease in blood levels of PlGF and increasing autoantibody monoclonal to the protein p53), allowing to offer new additional risk factors that can be used from early pregnancy. Mathematical evaluation of informative indicators, forecasting algorithm occurrence of preeclampsia. Improved range of preventive measures for pregnant women at high risk of preeclampsia, aimed at eliminating endothelial dysfunction.
