Essay

The research is devoted to the actual Surgery problem – improve the functional outcome of patients with occlusion of the internal carotid artery by surgical methods to prevent cerebral ischemia. External carotid endarterectomy is a safe intervention with 3,4 % of postsurgical ischemic complications. Factors that have influence on development of postinterventional strokes are: contralateral internal carotid arterу stenosis more than 70 %, intraluminal shunt utilization and history of ipsilateral disabling stroke. Patients who underwent surgery have less risk of ischemic stroke and ipsilateral transient ischemic attack comparing with only medicationally treated patients. Retrograde blood flow in ophthalmic artery creates hemodynamic conditions that improve blood supply of the brain. Factors that affect the utilization of intraluminal shunt during surgeries on contralateral internal carotid artery are retrograde direction of blood flow in ophthalmic artery, more than 70 % stenosis of internal carotid artery, the interval between the two surgeries and presence of Willis circle loop opening in two or more areas.
