The work deals with the reduction of perinatal morbidity and mortality in women with hemostasis disorders and threatened preterm birth based by the selection of high risk women and disease and administration them an individual correction scheme. The main risk factors, especially complicated personal and family thrombotic, common and reproductive anamnesis (miscarriage, stillbirth, early neonatal mortality, habitual miscarriage, failed attempts of IVF), are determined, using these signs prognostication of heamostasis disorders in patients with threatened preterm labor was performed. Basing on a complex evaluation of the hemostasis system and trombophilia laboratory criteria it defined the leading role of inherited trombophilia, hyperhomocysteinemia and antiphospholipid syndrome in genesis of preterm labor. According to the study results, for the first time, an individual pathogenetic based complex for trombophilic disorders correction in pregnant women with threatened preterm labor was worked out. It includes heparinoids, low-molecular heparin, omega-3 fatty acids, folic acid, group B vitamins and is controlled by markers of thrombophilia. The proposed trombophilic desorders correction complex allowed timely decision about further pregnancy prolonging and management in these patients. A new clinical approach to the genesis, development and clinical manifestations of the threatend preterm labor, caused by haemostasis disorders, designed and implemented complex of preventive and treatment measures made it possible to reduce the frequency of pregnancy complications, perinatal morbidity and mortality.
