Object - stage and the prevalence of purulent inflammation of the peritoneum through urgent abdominal pathology; goal - increase the effectiveness of sur-gical treatment of purulent peritonitis (PP) by predicting the course of early diagnosis and its complications, the objectification of the indications for surgical correction and retransmission of differentiated choice of medical tactics by the observation of the dynamics of immune-cytokine profile (ICP); methods - clinical, laboratory, biochemical, pathologic, tools, statistical; results - the degree of the impact of changes in the ICP and the functional status of the perito-neum macrophages in the postoperative course, the frequency of perioperative complications and results of surgical treatment of patients with PP, depending on the stage and the prevalence of peritoneal process; the features and patterns of change in the ICP indicators and the degree of dysfunction of peritoneal macrophages in patients with PP, depending on the stage and the prevalence of peritoneal process, the presence of postoperative organ and septic complica-tions; first established the reversal of ICP disorders and functional activity of peritoneal macrophages in patients with PP, depending on the composition of the perioperative medical therapy, surgical techniques and tactics of the factors influencing the rate of elimination of these changes; proven ability to use certain in-dicators ICP and associated cytopathic hypoxia markers and endotoxemia as diagnostic tests differentiation stage PP, prognostic predictors of post-operative tendency to progression, complicated and uncomplicated postoperative course, organ and septic complications; it offered pathogenetically substantiated targeted correction of the ICP changes and related metabolic disorders, depending on the projected preoperative risk of postoperative complications PP; branch - medicine
