Essay

The scientific work is based on the results of diagnostics and treatment of 90 patients with esophageal diverticulum different locations, including Zenker's diverticulum - 56 (62.2 %), Epiphrenic diverticulum - 25 (27.8 %), Bifurcation diverticulum - 9 (10.0 %). Disease duration ranged over a wide range - from 2 months to 40 years, an average of 3,7 ± 0,2 years. Leading clinical symptom was dysphagia - 72 (80 %) patients. Evaluation of the main clinical symptoms in patients with Zenker's diverticulum had showed that their frequency increased and significantly correlated with the duration of history of disease: dysphagia III-IVst. - r = 0,68; the asymmetry of the neck - r = 0,56; burp - r = 0,55; loss of body weight - r = 0,51; with epiphrenic diverticulum - with dysphagia (r = 0,57) and heartburn (r = 0,54). Also, with the duration of disease history significantly positively correlated the severity of inflammatory changes of the mucosa of Zenker's diverticulum (r = 0,46, p = 0.001), and according to the analysis of the dispersion - with the length of Zenker's diverticulum (R2 = 0,458) and epiphrenic diverticulum's length (R2 = 0,530), what the evidence in favor of the earlier surgery. The developed method of suturing the stump of diverticulum with an additional encircling stitch of mucosa over the line of staples seam had helped to reduce the incidence of insolvency seams stump of diverticulum in the early postoperative period, from 9.1 % to 0 %, p = 0.033, including its epiphrenic localization 22 2 % to 0.0 %, p = 0.049. Сricopharyngeus muscle myotomy with open diverticulectomy in patients with Zenker's diverticulum provides significantly better results in the cumulative two-year prospective study: 92.9 % good, 7.1 % satisfactory in the absence of bad results against respectively 70.6 %, 25.2 %, and 4, 2 % in patients who have not been performed myotomy, p = 0.032. The introduction of an improved approach for perioperative management of patients, the use of techniques developed operations, performance cricopharyngeus muscle myotomy (with Zenker's diverticulum), one-step removal of diverticular disease complications and simultaneous improved postoperative course and possible to achieve good (90.7 %) and satisfactory (9.3 %), results.
