Object - development of fetus distress in pregnant women with gestational and chronic pyelonephritis; goal – reduce the incidence of fetal dis-tress in pregnant women with pyelonephritis by developing pathogenetically based system of treatment and preventive measures aimed at correcting hyperhomocysteinemia and its associated metabolic and hemodynamic disturbances in placental system; methods – clinical, instrumental, biochemical, immune-enzyme, statistics; results – shows that women with gestational pyelonephritis recorded a significant increase in indicators of vascular resistance in the uterine and umbilical arteries, accompanied by the development of fetal status, while at the same time, in pregnant women with chronic pyelonephritis changes in these indicators are more ambitious; for the first time proved that with gestational and chronic pyelonephritis marked hyperhomocysteinemia, elevated levels of C-reactive protein and interleukin-6, increased lipid peroxidation and oxidative modification of proteins associated with a decrease in inventories of reduced glutathione, the formation of the imbalance in the pro-antioxidant system abnormalities in the L-arginine / NO; the formation of the imbalance in the pro-antioxidant system, disturbances in the system of L-arginine / NO; the formation of fetal distress and an increase in serum homocysteine levels in pregnant women with pyelonephritis increases the magnitude of the changes in these biochemical parameters; the first time it is shown that in pregnant women with gestational and chronic pyelonephritis predictors formation dis-tress fetus is homocysteine, interleukin-6, restored thiol groups, L-arginine and S-nitrosothiols in blood serum; branch – medicine.
