Shvets I. Justification use of intra-abdominal laparoscopic hernioplasty technique in the treatment of umbilical hernia (clinical and experimental study)

The aim of the dissertation is to improve the results of surgical treatment of umbilical hernia. The results of the pilot study in 36 rabbits after placing mesh implants through techniques "sublay" and IPOM and surgical treatment of 82 patients with umbilical hernia, which was carried out by methods hernioplasty "sublay" and IPOM. The proposed method improved intra-abdominal laparoscopic hernioplasty for umbilical hernia surgery (ІРОМ). The experiment found less pronounced macrophage infiltration and lymphocytic tissues in modeling techniques ІРОМ (p = 0.002), which prevents the overproduction of connective tissue and improves the integration mesh implant in the anterior abdominal wall. Strength of complex "implant-tissue anterior abdominal wall" in modeling techniques ІРОМ is comparable with those of strength placing the grid method "sublay". The introduction of improved methods IPOM for treating umbilical hernia has reduced the risk of postoperative complications (seromas origin from 12 to 0%), reduced to 1 day period of stay of patients in hospital and reduce the period of rehabilitation, creates a better cosmetic effect.
