On the basis of retrospective analysis of 126 patients, morphometric proven by increasing the volume fraction vascular component, increase in vascular microcirculation by dilation of blood vessels, which in combination with the identified structural changes and a predominance uricinase type of plasminogen activator inhibitor over it, which plays a major role in the occurrence of bleeding complications. The tactics of choice of endovascular interventions, which allows you to define indications for splenic artery embolization execution in patients with intrahepatic and extrahepatic portal hypertension based on the dynamics of pressure in the portal vein. Developed diagnostic algorithms treat bleeding from esophageal varices depending on the portal unit. Proved the use of endoscopic and endovascular therapies as complementary. Diagnostic algorithm leads to improved results of surgical treatment compared with traditional methods, thus reducing the incidence of rebleeding from 23,1% to 16,4%, the overall mortality rate - from 24,6% to 13,1%..
