The dissertation is dedicated to the improvement of the results of surgical treatment of the patients with complicated forms of varicose disease of lower extremities in the way of the method of surgical treatment and local therapy of venous trophic ulcers advance. The dissertation presents results of treatment of 148 patients with complicated forms of varicose disease of lower extremities. The main group consisted of 78 patients the treatment of which was fulfilled using method of subfascial endoscopic perforator vein surgery. The comparative group consisted of 70 patients who were subjected the intrusion from mini-accesses on perforant veins. Introduced thrombosed segment to remove a large segment of the great saphenous vein in the thigh by tunneling under endoscopic control method for the treatment of patients with venous trophic ulcers on the background of varicose veins by surgical treatment of venous hemodynamics and local application of platelet-rich plasma. Justified of active surgical tactics and the sequence of stages of surgical intervention in patients with varicothromboflebitis combined with venous trophic ulcers. Рroved the effect of platelet-rich plasma in combination with a radical surgical treatment of venous hemodynamics in the histogenesis of trophic venous ulcers, which manifests itself in the revitalization of physiological processes with the formation of a full repair of connective tissue. Proved high efficiency of the proposed methods and surgical local treatment of patients with complicated forms of varicose veins is based on the reduction of postoperative complications and improve quality of life indicators in accordance with the questionnaire "CIVIQ-20", as well as reducing the severity of venous dysfunction as assessed using VCSS scale.
