The dissertation is devoted to the improvement of the results of surgical treatment of patients with perforated duodenal ulcer using minimally invasive laparoscopic technologies. Development and implementation of methods of laparoscopic operations in patients with perforated duodenal ulcer allows using them in almost all age groups of patients, choosing the most suitable type of laparoscopic surgery, depending on the diameter of the perforated hole, presence of ulcer on the back wall of the duodenum, peritonitis or choosing traditional open technology surgery. The selection algorithm of minimally invasive laparoscopic surgeries was made on the basis of the proposed technologies. It gives the possibility to reduce the number of postoperative complications by 8,4% (by 5,4 times) and improve the results of surgical laparoscopic treatment of patients with this pathology in comparison with open surgeries. The appropriate personalized approach in choosing the surgical method and the method of laparoscopic treatment of patients with perforated duodenal ulcer in combination with the use of proton pump inhibitors in postoperative period provides an opportunity to prevent recurrence of duodenal ulcer and to get better treatment results on Visick scale for 7,9%, 10,5%, 9%, та 7,9% more, i.e. 3, 6, 12, 24 months respectively in comparison with traditional open operations.
