Essay

In the dissertation the author has demonstrated that patients with type 2 diabetes complicated by DPN have a mixed type of nerve fiber involvements with predominance of axonal degeneration. The secondary demyelization is observed with progression of the polyneuropathy severity. It happens owning to the worsening of the axonopathy process and, probably, due to immune disorders, the level of which is correlating with the degree of damage to the myelin sheaths of the motor fibers of peripheral nerves in the lower limbs.the significance of changing such parameters as cellular and humoral components of the immune system, markers of immune inflammation TNF-alfa, IL-1beta, complement components C3, C4, marker of damage to the myelin of peripheral nerves PMP22 in the polyneuropathy progression. It is established that the level of antibodies to РМР22 in patients with type 2 diabetes significantly increase with progression of DPN on the grounds of reducing the nerve excitation speed, which is a marker of the functional state of the nerve fiber myelin. It is proved that a complex therapy on the ground of basic treatment of patients with type 2 diabetes complicated by DPN may introduction the regeneration and remyelinationof the peripheral nerves.
