The thesis is devoted to solving actual scientific problem, namely, to increase the safety and efficiency of anesthesia and intensive care in the surgical treatment of patients with phlegmon of the neck by improving the algorithm of actions anesthesiologist while ensuring the airway in intra- and postoperative period. The paper analyzed the medical records of 75 inpatients in phlegmon of the neck. The thesis studied in detail the nature and frequency of perioperative complications in patients with phlegmon neck. It was established that the main cause of severe tracheal intubation failed and when a phlegmon of the neck is the presence of limitation of mouth opening and laryngeal edema. Efficiency videolarynhoskop Flaplight, fibrobronhoskop and tracheostomy for secured airway and their impact on the incidence of complications. For the first time udoskoneleno laryngoscope Flaplight adapter and proved its effectiveness. Installed effectiveness deksmetamedynu for comfortable prolonged intubation in patients in the postoperative period. It is proved that early tracheostomy in patients with neck phlegmon contributes to severe septic complications. Developed and scientifically algorithm of actions anesthesiologist while ensuring the airway in intra- and postoperative period in patients with phlegmon of the neck, which reduced the incidence of serious intubations with 51,4% and 7,9% respectively (?2=17,08; Р<0,01), failed intubations from 19% to 0,3% (?2=7,93; P<0,05), mediastinitis from 27% to 2,6% (?2=6,70; P<0,05 ), deaths from 32,4% to 2,6% (?2=11,62; Р<0,01) and mortality due traheostomiy from 32% to 6,3% (?2=8,52; P<0,05).
