Lopatenko D. Improving the results of surgical treatment pneumoempyema with mini-invasive technologies.

The dissertation presents results of a study of 159 patients with nonspecific infectious lung destruction complicated by pneumoempyema. It is proved high efficiency of the thoracoscopic surgery in combination with bronhoblokation and draining of lung-abscess cavity in patients with pneumoempyema, resulting in decreassng postoperative complications, length of hospital treatment and mortality. The study revealed some new immunological (IL6, IL8, IL10) diagnostic criterion effectiveness of treatment pneumoempyema and formulas for predicting death-resalt and changing treatment. It was designed method of draining lung-abscess cavity for sanitation lung abscess in the postoperative period. It was proposed and implemented method of visualization the pleural cavity with bronchoscope in case of intensive adhesive process. It was studied species composition of flora and determined sensitivity of selected strains to antimicrobial agents. We proposed scheme starting antibiotic therapy in patients with pneumoempyema wich based on the data of our researches. It was made comparative analysis of closed drainage and endoscopic sanitation of pleural cavity by the number of complications, duration of treatment and mortality.
