Chernyayev M. Surgical treatment of patients with liver injury taking into account of its venous structures.

Dissertation is dedicated to improve the results of surgical treatment of patients with liver injuries by individualization of surgery tactics using videolaparoscopic, angiographic technologies, and technology of surgical welding. Experimental and clinical research has been done. In the experimental section of 60 rabbits that were randomized into 9 groups (n = 6), in the comparative aspect studied morphofunctional status of liver tissue and vascular component in the province of resection after using the device high-frequency electrosurgical welding in manual and automatic modes, and monopolar electrocoagulation in different limitation periods from the time of surgery (1, 7, 21 days). In the experiment, it was proved that the most traumatic method is monopolar electrocoagulation, and the least traumatic - electrosurgical welding in manual mode. In the clinical section of analyzed results of treatment of 110 victims. The result was developed and introduced in clinic medical algorithm, the use of which has improved the results of surgical treatment of patients with injuries of the liver, reducing overall mortality from 13,7% to 8,5%, reducing the number of complications in the postoperative period from 27,4% to 15,3%, reducing mortality of patients with severe liver injuries involving damage to its large venous structures from 62,5% to 36.4%.
