In the dissertation the new solution of the actual task of modern obstetrics and gynecology is given - to reduce the incidence of reproductive disorders in women with septata uterus (SU). On the basis of a retrospective analysis of the patient history of patients who filed complaints of reproductive disorders and suspected congenital uterine defect, a diagnostic significance was found in the diagnosis of modern methods of instrumental uterine examination. The study of the expression of steroid receptors and pinopodes, the activity of proliferation, apoptosis and immune reactivity of the endometrium on the day of the proposed implantation window, the determination of the intensity of collagen formation in the tissue of the membrane, the features of peristaltic activity of myometrium in the dynamics of the menstrual cycle in non-pregnant women with ER, as well as features, apoptosis, proliferation, immune reactivity, angiogenesis and expression of mRNA of angiogenic factors and metal-matrix proteinases in different departments of the decidua of women with SU and spontaneous abortions allowed to develop complex perioperative drug therapy and improve reproductive results both in unipolar and bipolar hysteroscopic metroplastics. Installed that the onset of pregnancy after the operation does not depend on the type of technique of metroplastic - mono- or bipolar. But the use of bipolar metropolystic compared with monopolar one leads to an increase in the termination of pregnancy by emergency delivery in 1,34 times (OR = 2,6 ± 0,5; 95 % CI: 1,0-6,3) and with the additional use of perioperative pharmacotherapy - in 1.45 times (OR = 3.0 ± 0.6; 95 % CI: 1.0-9.1).
