This thesis is devoted to the study of reproductive health condition of men with infertility in marriage and devising medical measures for its improvement in the aspect of preventing development of male infertility and improving primary medical care provided to couples and, first of all, men with infertile marriage.The condition of reproductive potential was studied in 1265 men from Kyiv, 11 oblasts of Ukraine and the Autonomous Republic of Crimea.It has been proved that andrologic component of infertility in Ukrainian population reaches 47,5 %, i.e. this figure is much higher than the official data. It has been established that the most prevalent and considerably widespread factor of generative function disorders in men are infectious-inflammatory diseases of genital organs, diagnosed in 35,5 % of the cases. According to the studies, frequent causes of reproductive disorders include varicocele (15,0 %) or its combination with other non-inflammatory lesions of urogenital system organs (10,4 %), pathology of one or both testicles - incurred orchitis, including the one associated with epidemic parotiditis, late-operated cryptorchidism, testicle removal due to its injury or torsion, pronounced hypoplasia of both testicles of uncertain origin (11,3 %), unhealthy lifestyle (8,2 %). It has been shown that male fertility disorders are also frequent in couples with infertility deemed to be caused by tubular-peritoneal pathology in women. They occur in at least 33,6 % of individuals. The main cause of decreasing reproductive potential in these patients is infectious-inflammatory pathology of urogenital system (51,2 %). It has been established that decreased generative function is observed in 23,5 % of men planning childbirth. Its causes are the same as in individuals with infertility in marriage in the population. The number of patients seeking a doctor's consultation due to planned childbirth is 6,8 times lower than that seeking medical assistance in connection with the problem of infertile marriage. It has been determined that the quality of primary medical health care provided to men with infertility in marriage is low.Medical measures for prevention of reproductive health disorders in men, aimed at close monitoring of men's health condition at different stages of their lives beginning from birth, early identification and timely treatment of pathology, have been scientifically substantiated and devised. Improvement in organization of primary medical care provided to couples and, first of all, men with infertile marriage, has been suggested. Wide-scale implementation of these measures into healthcare practice should help improve reproductive health of men and decrease infertility in marriage.
